To complete the Secret of the Spring side quest in Diablo 4, you must head to the northeast of Kyovashad in the Fractured Peaks zone. Before you can begin the quest, you must find a Discarded Note, which can be found on the ground. The note is described as "a crumpled note left on the ground."
Once you've interacted with the note, you'll be instructed to go to the small hot spring.
